The most important thing you can do is to start thinking about, discussing and planning for the future. Don't worry though, you won't be alone with this. 

To do this you will need to know what options are available.There is lots of information available to help you to prepare for each stage of the transition process. Information and advice is readily available to everyone on the Kirklees Local Offer webpage.

Support is available to explore your options if you would like it. Specific information will also be provided at key times throughout the pathway. These are broken down into the sections below.

Preparing for Adulthood/Preparing for the future best practise guide

Who is it for?
  • Professionals: schools, colleges, training providers, social care, and health teams.
  • Parents & carers: to help them navigate choices and encourage independence.
  • Young people: to empower them in shaping their own future.
Why is it important?
  • To ensure that young people and families have the right information at the right time.
  • To support well-informed decisions about education, health, care, and future life goals.
  • To make sure the transition into adulthood is coordinated, supported, and reviewed regularly.
Who should be involved?
  • Young person (always at the centre)
  • Parents/carers
  • Education/training provider (school, college, or specialist setting)
  • Careers adviser statutory access up to 18.
  • Social worker (if applicable)
  • Health professionals (e.g., paediatrics, GPs, therapists, CAMHS/adult mental health services)
  • Other key workers or advocates actively supporting the young person
  • If you have an EHCP, the case officer / local authority SEND team may be involved.
